    Assisted reproduction techniques help to solve problems of fertility and sterility. Using a variety of processes which stimulates the gametes (eggs and sperm), these techniques aim to result in fertilization and enable the transfer of embryo to the uterus.

    Spain offers an excellent fertility service as its legislation is more flexible than in most other European countries, and its success rate for in vitro fertilization and egg donation is also high. In addition, quotes for performing these treatments are more affordable in Spain than in other European countries.
    The emergence of the In Vitro Fertilization, more than 30 years ago, has changed the approach used to treat infertility and has contributed to the development of complementary technologies which have improved the results of conventional IVF. Moreover, there is now more significant knowledge about the causes of infertility.

    > IN VITRO FERTILIZATION – IVF

    In Vitro Fertilization (IVF) is the union of the egg and the sperm in the in vitro laboratory. The objective is to obtain embryos that can be implanted into the uterus.
    This technique, developed in a state-of-the-art laboratory, is used when previous treatments have failed, such as artificial insemination. In addition, this treatment is also a good solution when the male is suffering from fertility problems or when the female has blocked fallopian tubes – amongst others.
    The fertilisation of the ova can be carried out through the conventional IVF technique or with an Intracytoplasmic Sperm Injection (ICSI). This is used in cases of severe male infertility, fertilization failures prior to IVF, the failure of previous artificial insemination treatments or situations where there is a limited number of oocytes. This technique is used to select and extract the best sperm to fertilize the eggs.

    In Vitro Fertilization (IVF) can be performed by either using the couple’s semen or donor sperm.

    IVF Treatment:
    Customised Assessment for each patient
    A customised diagnosis is performed for each patient so that the best form of treatment can be selected for their fertility problems. For this reason, family medical records, body mass index, ovarian reserve, etc. are examined.

    Ovarian stimulation
    Ovarian stimulation during IVF treatment consists in daily injections that stimulate the ovaries so instead of producing a single egg -what spontaneously occurs each month-, produce more than one to be able to get a greater number of embryos.
    This treatment is performed in the IVI reproduction clinic and lasts from 10 to 20 days, depending on the protocol used and the speed of response of each patient. During the treatment a series of ultrasound scans will be performed (between 3 and 4) and the levels of estradiol in the blood will be checked to ensure the correct growth and evolution of the follicles.

    Follicular Puncture
    When the ultrasound scans show that the follicles have reached the appropriate size, and there are sufficient eggs available, we schedule a follicular puncture 36 hours after administering an hCG injection that induces the oocyte to mature. The follicular puncture is performed in an operating theatre under sedation, so that the patient does not feel any discomfort during the procedure which lasts approximately 10-15 minutes.

    In vitro Fertilization
    Once a follicular puncture has been performed and sperm has been taken from a sample of the couple’s semen, the egg will be inseminated. This can be done either by using the conventional IVF technique, which consists in placing an egg surrounded by sperm on a culture plate, or by the ICSI technique, which introduces live sperm into the egg by puncturing it using a pipette.

    Embryo culture in the laboratory
    The resulting embryos obtained after in vitro fertilisation, are observed in the lab day by day, and are classified according to their morphology and capacity for division. Some embryos may become blocked in their development, and will therefore be discarded as not viable.

    Embryo Transfer
    This treatment allows for the introduction of the best embryos of the group into the maternal uterus with the use of a cannula specially designed for this purpose. The procedure is performed in an operating theatre, although, in this case, sedation is not necessary as it is a quick and painless process.
    Cryopreservation : Vitrification of embryos
    Once the embryo transfer is performed, the remaining good quality embryos are vitrified so that they can be used in a subsequent cycle without having to stimulate the ovaries again. Vitrification is giving encouraging results as the embryos are not affected by it and the pregnancy rate does not decline -as in the case of the slow or conventional freezing – in relation to the rate obtained with the fresh embryos.

    Pregnancy Test
    If the test is positive, a vaginal ultrasound should be performed a week later to display the gestational sac.

    The entire procedure of endometrial preparation can be done in your country of origin under our supervision. You will only need to go to IVI cater for the follicular puncture and the embryonic transfer.

    > IVF USING EGG DONATION

    Egg donation is the process by which a woman uses eggs from a donor in order to become pregnant. The donor eggs are united with the couple’s sperm to form the embryos. The resulting embryos are then implanted into the female. In this way, egg donation makes the miracle of life possible for women who would otherwise not have children.

    The egg donor
    Egg donation was legalised by the Spanish legislation bodies in 1988 (Law 35/1988, 22 November). The donor must meet the following requirements to be able to donate eggs:
    • Age: 18 to 35 years
    • A genetic history according to the requirements of current legislation
    • Normal functioning of the reproductive tract
    • Complete gynaecological evaluation
    • Complete psychological evaluation
    The choice of the  egg donor is made on the basis of the phenotypic characteristics of the couple and the donor (height, size, hair colour, blood group, etc.).

    The receptor
    The following groups of women will be candidates for the egg donation programme:
    1.    Women with ovarian failure without spontaneous menstruations due to menopause, premature ovarian failure or ovarian surgery. Primary ovarian failure occurs when menstruation does not actually happen during puberty and premature ovarian function failure is when the ovarian function fails before the age of 40 with periods that begin to be irregular until they finally disappear. Amongst these causes are hereditary factors, cases whereby the patient has suffered an infection, having been subjected to chemotherapy or radiation therapy, etc.

    2.    Women with functioning ovaries that are not be able to use their own oocytes either due to; the poor quality of their eggs or to hereditary diseases that can be transferred to their offspring (which cannot be detected by Genetic Pre-Implantation Diagnosis techniques). It is therefore advisable to take advice before conceiving to clarify the risks of transmitting an autosomal or chromosomal disease to any offspring, as well as gender related condition such as haemophilia.

    3.    Women with repeated failures using in vitro fertilization (IVF):
    • Low response: women who do not respond to ovarian stimulation.
    • Repeated fertilization failure using ICSI.
    • Repeated embryos implantation failure (repeated gestation failure).

    4.    Other cases in which the use of donated eggs is recommended is when women have ovaries which are inaccessible for obtaining oocytes, if she has suffered repeated miscarriages, when there are chromosomal abnormalities in the woman or the embryos, women over the age of 40 with a very low probability of pregnancy or high risk of Down syndrome, etc.

    What should women or couples bring with them if they want to be the recipients of oocytes?
    • Prior Medical and gynecological history
    • Report of prior treatments in other centres
    • Updated serologies of hepatitisB and C, syphilis and Aids for both parents
    • Blood groups and RH of both parents
    • Complete and sign the request and informed consent for fertilisation.

    The egg donation treatment
    The recipient of the embryos should receive a treatment to prepare the endometrium before the embryo is transferred.

    Endometrial preparation for the patient
    This consists in administering a hormonal therapy with the aim of preparing the endometrium for the implantation of the embryos. For this reason, it is necessary to halt ovarian activity and, subsequently, administer oestrogen through transdermal tablets or patches in order to help the proliferation of the endometrium. Once the adequate thickness and conditions are obtained and the date for the donation is set, the progesterone (the hormone that the ovary produces under normal conditions after ovulation), starts to be administered.

    Follicular puncture of the donor
    Once an ultrasound has confirmed that follicles have reached the appropriate size and that a sufficient number of ova are available, the follicular puncture is programmed around 36 hours after the administration of an injection of HCG hormone that stimulates the eggs to mature (as they would in the natural cycle).
    The follicular puncture is performed in an operating theatre and under sedation, so that the patient does not feel any discomfort during the procedure, which takes approximately 15 minutes.

    In vitro Fertilization
    Once we have the eggs, after the completion of the follicular puncture, and sperm, which are normally obtained from the collection of a sample of the couple’s semen, we will proceed to inseminate the eggs.
    This can be done either by using the conventional IVF technique, which consists in placing an egg surrounded by sperm on a culture plate, or by the ICSI technique, which introduces live sperm into the egg by puncturing it using a pipette.
