Treatments to correct vision

There are a variety of effective surgical treatments to correct vision defects, whether they are defects due to curvature or characteristics of the eye, such as myopia, hyperopia and astigmatism, or due to aging, as in the case from presbyopia or tired eyesight.

In this article, Dr. Cilveti, Head of the Ophthalmology Department at Vithas Xanit Internacional, summarizes us what are the different surgical treatments available to correct vision without the need for glasses or contact lenses, and in which type of cases one or the other are usually used.

Laser surgery treatments

Currently laser surgery can correct any of the visual defects: myopia, hyperopia, astigmatism and presbyopia. These are outpatient treatments, so it takes only about 20 minutes to perform them quickly and painlessly, after which the patient only needs to rest for a while and, after verifying that everything is correct, can go home the same day.

The most used technique is LASIK, which separates a thin layer of the cornea (which is carried out, depending on the variant of the LASIK technique used, by means of femtosecond laser without cuts, or by means of a surgical cutting tool called a microkeratome) to be able to apply the laser just below, reshaping the shape of the cornea to correct visual defects, finishing the procedure by putting the separated layer back in place.

In the case of LASIK correction of presbyopia, remodeling of the corneas can be performed using two different techniques:

  • With the “Monovision” technique, one eye is corrected to see up close and the other to see from a distance, so that the brain chooses at all times the image of which eye to use. This technique requires adaptation time for the patient to get used to it.
  • On the other hand, using the “PRESBILASIK” technique, the cornea is modeled in both eyes as if it were a multifocal lens, with an area to see up close and another to see from afar, being the eye the one that decides through which zone to see according to the distance to focus, which has important advantages over Monovision.

Finally, the PRK technique is also used, which allows treating patients with thinner corneas, and does not require separating any layer of the cornea, reason why with this technique it is usually the preferred technique in the case of athletes, sportsmen and other risk professions, since in the event of accidental trauma in the future, there is no risk that this layer may come off again, which could cause complications. The downside of PRK is that recovery is slower and somewhat less comfortable compared to LASIK.

The results after the complete recovery of both laser techniques (LASIK and PRK) are similar.

Treatment by implantation of intraocular lenses

Another alternative to laser treatment is the implantation of intraocular lenses. These lenses are implanted inside the eye, replacing the crystalline lens, through a surgery whose procedure is basically the same as that used in cataract surgery.

These intraocular lenses can be graduated to correct any of the vision defects, and can be an alternative in those cases in which laser surgery cannot be performed even with PRK. In the case of presbyopia correction, multifocal intraocular lenses are used.

As expected, surgery to implant intraocular lenses is more complex than laser treatment, although it has the advantage that the same surgery eliminates cataracts (or prevents them from appearing in the future), since the crystalline lens, whose aging and opacification is the cause of cataracts, is replaced by the intraocular lens.
